Transitional Care
Phase Two of Your Journey
Welcome to the Alpine Treatment Residential Transitional Care Facilities located in Midway, Utah, nestled in between Sundance and Park City. Our facility provides an ideal platform as individuals transition from phase one to phase two of our program.
Transitional care is an instrumental component to the recovery process. It offers an opportunity for individuals to practice the skills and techniques attained during the inpatient portion of the Program, while having the freedom to re-gain relationships and learn new ways of living. Having these core elements integrated into the Program is an innovative and vital feature.

Dynamic Program and Services Provided
Clients will have the opportunity to participate in many of the of the services provided by The Chateau, including: intensive cognitive therapy by licensed, professional therapists; 12-step counseling by licensed substance abuse counselors, various therapeutic groups & workshops, drug and alcohol screening, our Family Forums, access to the gym, recreational therapy, a dietitian approved meal plan, and a dynamic team of professionals to support individuals on their journey.
One of the most exciting factors of our Transitional Care Program is that our clients are encouraged to work and pursue educational and/or vocational goals. These experiences promote a real world experience where individuals are able to pursue such goals while remaining sober and supported.
Prepared for Your Future
As the recovery process continues, individuals receive continued support and guidance to gain necessary tools for this level of care. It is an opportunity for individuals to prove to themselves, and to others, that they have the ability to maintain a healthy, balanced and sober lifestyle. They are given the proper amount of freedom to test the waters, while having a safe place to call home.
Phase Two is trust-based, with the belief that trust is the greatest human motivator. Trust is re-enforced through the program in a variety of ways. The importance of a caring staff to provide guidance regarding the balance of freedom and accountability is essential. As trust is proven and earned, a new confidence comes over our clients as they step toward independence and learn to trust themselves. Such an element is essential for long term sobriety.
Set Up for Success
Our Transitional Care Program offers a unique experience where our clients are able to take the time and space needed to practice what they have learned. The intention of Transitional Care is not to pull individuals out of their families or current situations completely. It is necessary to take time needed to re-build relationships that have been damaged, and allow them to slowly regain their lives and independence. Transitional Care offers a “real world” experience with the structure and support needed for long-term sobriety.
